An AND Gate? There's not really much you need to "build up", you can just throw a single chip in there like a 7408 (Which is overkill for this as you don't need to work with that many gates but it's all I can remember right now). You could wire Everything up through a project box, and add the PS button there. probably the best way to add buttons without drilling holes. Or replace the PCB currently in the stick with one that supports Start+Select=Home like the PS360+, or the Cthulhu.

Most of the custom builders over on SRK will make a case with whatever layout you want. I would expect a case like that to run upwards of ~$120-$150 (for just the case), maybe a bit cheaper depending on options. For the entire thing to be wired up, with buttons and a pcb would depend on the stick builder (if they even offer that option.)
